Decisions will be sent by email only sometime in the month of February for a fall application and sometime in the month of September for a spring application. USC Upstate cannot be held responsible for unreceived email communication. If you have not received a decision email by the end of February or September (depending on the term for which you applied) you need to contact the School of Nursing at 864-503-5441.

Statements of Understanding

I understand that admission requirements state that my cumulative GPA should be a minimum of 3.0 on a 4.0 scale and that my science GPA must be a minimum of 2.5. It is preferred that this science GPA be on at least three of the five natural science classes, but two must be included in the admission GPA. Students with less than a 3.0 cumulative GPA may be considered and accepted on a conditional basis if space is available.

I understand that only one nursing course or required science course may be repeated to earn a grade of "C" or better. If courses are repeated, both grades will be included in the calculation of the GPA. A student with two failures in nursing and/or science courses is ineligible for our program.

I understand that I must maintain at least a 3.0 cumulative and a 2.5 science GPA after the completion of all outstanding prerequisite classes.

I understand that grade forgiveness is not recognized by the School of Nursing. All grades will be used to calculate admission GPA's.

I understand that all required natural science courses must have been successfully completed within the last five years.

I understand that I am only allowed two attempts at each science/nursing course. Withdrawals and Audits count as attempts.

I understand that I have the option of completing the TEAS exam or submitting a resume’ and that the option I select on this application cannot be changed.

I understand that if I am taking courses at any other school than a USC campus, I must send all transcripts showing my final grades to USC Upstate Admissions as soon as the grades are posted. *If you are a USC student and have taken courses at another school and those courses are showing on your record in the Self-Service student portal, you do not need to send additional transcripts.

I understand that if I receive tentative acceptance to the program I will be required to complete a criminal record background check. If that report is not clear, I understand that tentative acceptance can be revoked.

Course Entry

Prior to admission into upper-division nursing courses, the following lower-division courses must be completed. Please list the courses that you have taken to meet these requirements and your plan for completing the remaining courses. ** Please note that only one summer course is allowed just prior to a possible fall admission. The course must be taken at USC Upstate or at another USC campus.

** Two of the five natural science courses must be successfully completed by the end of the term in which you are submitting this application, however three is preferred. Students must maintain a 2.5 GPA after completing all five courses. **
** CHM 105 or 111 from a SC technical college are the only acceptable SC technical transfer courses for our CHEM 109. CHM 110 is NOT the correct chemistry class. **

Checklist for Criminal Record Background Check (To be completed after acceptance as directed.)

If accepted, I will send in my information and fee for the Criminal Record Background Check (CRBC) to the CRBC company, and will sign to have the information released to the Dean of the Mary Black School of Nursing.

I give the Mary Black School of Nursing at USC Upstate permission to receive the results of my CRBC and to provide such results to clinical agencies for the restricted purpose of determining my suitability to participate in the clinical practicum courses in the Mary Black School of Nursing Baccalaureate Nursing program. The results of my CRBC may not be shared with any other entity without my express written permission.

I understand that I am to advise the Mary Black School of Nursing at USC Upstate of any arrests or criminal charges subsequent to my completing this form and that failure to do so may result in dismissal from the nursing program.

I understand that the Mary Black School of Nursing at USC Upstate will keep the CRBC reports in a confidential area, and that these reports will be maintained for 3 years following my last completed nursing course at USC Upstate.

I understand that if it is necessary to reapply for initial acceptance to the Mary Black School of Nursing at USC Upstate, the CRBC will need to be repeated. I further understand that if there is ever an interruption in my enrollment, I will have the CRBC repeated when I return to the nursing program.
